St. Ann’s Community (SAC), located in Rochester, New York, is a not-for-profit organization providing a varied range of services and care levels designed to meet the individual needs of older adults. St. Ann’s offers several independent and retirement living options, transitional care, Adult Day Services, Assisted Living, specialized memory care and 24-hour skilled nursing care, Hospice and end of life care.

The Internal Medicine/ Geriatrics Physician will serve as a physician within the Medical Staff providing clinical care to residents/patients in accordance with medical program policies and procedures including but not limited to: Admission H&P, Acute Visits, 30/60 Day Visits, and discharges.
The Physician will work collaboratively with the medical team to maintain the highest quality of medical services and continue achieving the organization’s goals. He/She will report to the Chief Medical Officer and serve as an advisor on matters of clinical care.
Qualifications include:

• MD/ DO with 1 to 4 years of clinical experience
• Board Certification in Internal Medicine or related field
• Hospitalist experience with emphasis on geriatric care is desirable, but not required
